Dear Mr. Monson,

This is to inform you that I am requesting my (our) name(s) be removed from the records of the Church of Jesus Christ of Latter-day Saints. (Include any Maiden name). And to help expedite this, I was born in (city, state) _______________________ and my birth date is M/D/Y.

It has been demonstrated to my satisfaction that Mormonism is in great error, that Joseph Smith was a false prophet and that the Book of Mormon is not from God. As I have studied Mormon history, read the Bible and prayed to God, my eyes have been opened to the deception of Mormonism and the love of Christ. Jesus is now the Lord and center of my life and I know no one goes to the Father except through Him. I rejoice that I have this assurance and joy of my salvation according to God's Word, The Holy Bible (1 John 5:10)

I will not participate in any Bishop's court as I am not guilty of anything, nor do I view the LDS church as having any authority over me. Jesus is my only judge.

In your letter communicating to me that these instructions have been carried out, I insist that there be no mention of ex-communication because this is my decision and not yours. If this action does not occur within 30 days or anything is done or said that libels my good name or causes me loss of reputation, I will proceed with legal action against the LDS church and I will contact the various local news media.

Please be assured that my decision has been the result of much prayer, and is final. I will consider any attempts by the Priesthood, Home Teachers, missionaries or any other representatives of the LDS church to contact me an infringement of my right to privacy and my free agency. I am aware of what the LDS church believes are the eternal consequences of my (our) decision.

Finally, I testify to you that Jesus Christ is the only mediator between God and man (1 Tim. 2:5) and the Mormon apostles, prophets and priests are false ministers (2 Cor. 11:13) and that Mormonism is the other (false) gospel that Paul warned of in Galatians 1:6-9.

Jesus declared, "I am the way, the truth and the life, no man cometh to the Father but by me" (John 14:6). I (we) have chosen this day to follow Him.
